I am not convinced that we need to include this particular corenr case in the release notes. If you both think that it's important, though, how is this paragraph as a description for the use of both <label> and label=?
When you add problems using OLX, be sure to use the ``<label>`` element instead of the ``label=`` attribute to identify the question or prompt text. Including both the ``<label>`` element and the ``label`` attribute in a problem can have unintended consequences.
.. note:: Only problems that you set up to include different input types within a single ``response``, such as a multiple choice problem that requires a text input as part of the answer choice, should use both the ``<label>`` element (within the response type) and the the ``label=`` attribute (for the input type element) now.
