Failing a learner in Proctortrack doesn't update override history correctly

Description

Failing a learner in proctoring with Proctortrack should capture the Grader and Reason when overriding the learner’s grade to 0.

ACs

  1. “Grader” is set to username of Instructor/edX staff account that failed the user (e.g. olgastroilova in this case)

  2. “Reason” is broken out by vendor:

    1. Verificient Proctortrack: “Failed Proctortrack proctoring”

Separate ticket

  1. PSI RPNow: “Failed RPNow proctoring”

Current Behavior

  1. “Grader” is set to “verificient”

  2. “Reason” is blank
