Some CCX coaches have managed to create two separate instances of a course tied to the same master course. We have been unable to reproduce the behavior as perhaps it is related to having multiple open tabs when trying to create the course. For master course, course-v1:BJCx-CCX+bjc12x-ccx+FA2016, user_ids 80151 and 132544 each have two unique courses.
User 80151 has courses ccx-v1:BJCx-CCX+bjc12x-ccx+FA2016+ccx@475 and ccx-v1:BJCx-CCX+bjc12x-ccx+FA2016+ccx@476
User 132544 has courses ccx-v1:BJCx-CCX+bjc12x-ccx+FA2016+ccx@587 and ccx-v1:BJCx-CCX+bjc12x-ccx+FA2016+ccx@588
Any idea how this can happen? CCX coaches are can only make one copy of a master course.
