Canvas has extremely intuitive authoring tools for Hotspot activities. However, their hotspot activities are not accessible and are avoided by most institutions with a legal duty for accessibility.
iSpring (who make quizzes for Powerpoint presentations) include the ability to designate multiple areas that can have individual scoring and feedback. They include the same shape options as Canvas (rectangle, circle, polygon). Their tools are very clearly not accessible, as there’s no support for alternative text, let alone any more advanced accessible tooling.
H5P has a Find-the-Hotspot activity, but it is once again listed as not accessible in their documentation, making it unsuitable for academic use. This still doesn’t stop people from using it. Multiple areas can be defined, with per-item feedback.
