Required Information for Certificate Issues

Motivation

In order to help us triage and fix certificate-related issues, we're asking those who file issues to include the following information in their tickets. Please, for each line item, include the relevant information or an explanation why that information was not available.

Information

Kind of certificate: (pdf or web)

Course: (link to certificates panel on instructor dashboard)

Students affected and their enrollment modes (i.e. honor or verified): (usernames or emails)

Links to the students' certificates' urls.

Dates of generation and regeneration:

Who generated and regenerated the certificates?

Were certificates self-generated or instructor-generated?

Course pacing: is the course self- or instructor-paced?

Who originally raised issue that resulting in this ticket?
